

The Australian Primary Care Collaboratives Program aims to improve clinical health outcomes, reduce lifestyle risk factors, maintain health for chronic and complex conditions and improve access to Australian general practice.
The first phase of the Collaboratives Program was announced in the 2003/04 Budget with funding drawn from the Focus on Prevention – Primary Care Providers Working Together and the Fairer Medicare – Improved Information and Practice Management initiatives.
The first phase of the Collaboratives Program, formerly known as the National Primary Care Collaboratives (NPCC) was extremely successful with over 40 Divisions of General Practice and approximately 600 practices participating nationally.
As part of the 2007/08 Budget, the Australian Government announced that the Collaboratives Program would be continued and expanded. The Improvement Foundation Australia was awarded the tender to deliver the second phase of the Collaboratives program in November 2007 and will commence management of the program in Jan 2008.
